13.562 Manager, Patient Navigation & Utliization (Full Time)

Job Summary

The Manager of Patient Navigation and Utilization reports to the Director of Organizational Effectiveness, Patient Safety and Quality. The Manager, Patient Navigation and Utilization provides leadership in the facilitation of patient processes to support effective patient flow, from admission through to discharge, supporting effective bed and resource utilization. The Manager will be accountable for ensuring that care requirements are met with appropriate competence and delivery, while implementing flow policies, principles and procedures. In this role, the Manager will be responsible for liaising with a variety of units/department and community agencies to facilitate efficient and effective patient transitions along the continuum of care.


  • Regulated health care professional holding current certification with a regulated college in Ontario required.
  • Minimum 2 years management/leadership experience required.
  • Sound knowledge base of the health care system required.
  • Minimum 3- 5 years acute care and or community experience required with experience in case management, discharge planning, utilization review or equivalent preferred
  • Post diploma education in Case Management or Utilization Management preferred.
  • Carries a Membership with Association of Discharge Planning Coordinators of Ontario or Utilization Manager Association of Ontario
  • French Language proficiency an asset


  • Oversees the daily bed requirements of patients to be admitted and/or transferred to Windsor Regional Hospital
  • Provides primary leadership and works collaboratively with Patient Flow team, Physician advisor and Program Administrative Leadership (Directors and Managers) to manage multiple flow priorities, up to and including utilizing escalation space if needed
  • Expedites the transfer of patients from one level of care to another, facilitating the interface between each program and area
  • Acts as a clinical resource to staff and bed allocation in problem solving patient flow issues
  • Facilitates early intervention with respect to discharge planning, while ensuring organization’s compliance with provincial legislation pertaining to discharge planning and flow of patients through the continuum;
  • Collaborates with community partners to provide a seamless service delivery system for patients requiring care inpatient care and post discharge care from the hospital.
  • Communicating information in an effective and timely manner through all available resources.
  • Assuming accountability for facilitating the resolution of identified patient care issues in collaboration with the Patient Care Team.
  • Other duties as assigned.
