It is very important that we have up to date and accurate contact information for your family and/or significant other and next of kin. Please confirm/verify that the information on file is up to date and any changes are communicated to staff immediately. We will also ask you for information about your Power of Attorney for both personal care and finances. We will need copies of these for your chart.
