Social Work/Discharge Planning Services
The primary role of the CCC Social Worker is to assist the patient and family in making appropriate decisions regarding placement when it is determined that the patient no longer requires the services offered in CCC. This may involve facilitating placement in a long term care facility, assisting with arrangements to plan a safe discharge home, or coordinating other community referrals as needed. The Social Worker may also be involved in providing supportive counseling to patients and/or families. The discharge planning process will begin within 48 to 72 hours after admission to CCC.
