Discharge Information

Patients who are deemed medically stable by the Physician will be discharged home, to a rest home, to a long term care facility or to a rehabilitative/community program that is suitable for their needs. Complex Care is NOT a permanent place of residence; once medical conditions have stabilized or improved further, our team will help determine a more appropriate care setting. The exception to this is for end-of-life care; these patients will stay with us unless they choose to go home.
