Our Purpose
The Geriatric Assessment - Consultation Program (GAP) is a comprehensive interlocking network of specialized services.
The primary goal of the program is to promote independence and to avoid institutionalization of persons over 65 years of age who are frail, at risk. or exhibit complex or multiple problems. This is done through the provision of specialized geriatric assessment, consultation and care planning services.
Aging affects people differently and a planned care program must meet these individualized needs. GAP works with family practitioners, community agencies and support services to develop an integrated approach to care planning.
Through the education of clients, family and caregivers, this inter-disciplinary approach facilitates a comprehensive, client centered outcome that can help maintain an independent lifestyle.

| Target Population
The GAP will focus on seniors over 65 years, presenting with multiple/complex problems requiring multi-dimensional assessment. They may be at risk for institutionalization, decreased independence and deterioration to higher levels of care. These problems are of medical, functional, psychosocial nature, such as:

multiple pathologies
-
symptom complexes
-
incontinence
-
confusional states
-
falls/immobility
-
polypharmacy
-
unexpected response to illnesses
-
unexplained loss of functional ability
-
weight loss of unknown cause
-
multiple losses
-
multiple visits of ER and /or multiple hospital admissions
-
escalating care needs/decline in independence

Regardless of the source of referral, the support and awareness of the family physician is an essential part of the intake process.
