Program Description
The Adult Day Program is an outpatient program for physically handicapped and/or disabled adults.
Program services are based on the premise that people maintain their physical and mental well being longer when they are living in the community as opposed to an institutionalized setting.
Activities are provided to allow clients to explore their values, interests and abilities and to be active participants in developing their own quality of life.
A variety of social and recreational activities as well as health maintenance, education and promotion services are provided.
Why Adult Day Program?

To fill in the gap in program/services for those clients who do not require traditional health care services but cannot participate in existing social/recreational environments because of personal care and assistance requirements.

To reduce homebound tendencies.

To provide opportunities for peer interaction.

To provide alternative respite for caregivers.
Services Offered
- Vital signs taken daily
- In house foot care and nail care
- Community outings
- In house entertainment and games
- Outdoor activities in a park-like setting
Population Served
Clients served by the Adult Day Program:
-
are 55 years of age or over
-
are physically handicapped and/or chronically ill
-
are in need of stimulation, group activities and socialization to combat isolation
-
are residents of Windsor or Essex County
-
have the capacity and willingness to participate in the program with supportive care
Referral Guidelines
Referrals are accepted from individuals, families, health care and community agencies and physicians.
An assessment will be completed on referred clients to determine eligibility for program.

Program Costs

Transportation arrangements and any expenses are the responsibility of the client and/or family.

Meals are not provided. Clients are encouraged to bring a lunch if necessary.

A nominal daily fee will be charged.
