Our Purpose

Residential Treatment FacilityThe purpose of the Residential Rehab and Treatment program is to assist with the community re-integration for persons with an extended in-patient history. 
This is a structured rehabilitation and treatment program for clients with severe and persistent mental illness, which strives to facilitate a return to their home community. Residents will develop and practice skills such as meal planning and preparation, shopping, budgeting, use of transportation, leisure, job readiness and personal coping.

Admission Criteria

Individuals from Windsor-Essex with severe and persistent mental illness who:

  1. Have been institutionalized for long periods of time (or)
  2. Have suffered repeated failed attempts at community living (or)
  3. Require intensive skill development and support to increase their level of independence

Service Description

  • Community and residential support offering 24 hour care on premises.
  • Partnerships with Regional Mental Healthcare - St. Thomas and the Essex A.C.T. teams of the St. Joseph’s Regional Mental Healthcare - St. Thomas.
  • Interdisciplinary staffing provided by W.R.H. consists of Registered Nurses, Registered Practical Nurses, Occupational Therapist, Recreational Therapist, Residential Attendants and Psychiatrists.
  • Two (2) year maximum length of stay.

Rehabilitative Process

  • Client's strengths and rehab needs are identified through a formal bio-psycho-social assessment proces
  • An individualized rehab plan is developed jointly with the resident.
  • Treatment includes intensive skill building in activities of daily living and interpersonal communication.
  • The Essex A.C.T. team joins the Residential Rehab Program early in the process of discharge planning which opens the final door to community living.

Referral Guidelines

A referral must be submitted from a long term mental health facility to the Essex County Access Committee. For more information please contact Leslie Davis, Coordinator, at (519) 258-2331. Persons referred are expected to attend the program for short-term trial visits in advance of entering the program. All referrals, which must also be eligible for Essex A.C.T. service.

Fees

Rehabilitation and treatment services provided by W.R.H. are funded by the Ministry of Health and Long-term Care, Province of Ontario. The Residential Rehab and Treatment Program has contracted with the Windsor-Essex County Housing Corporation to provide affordable housing for our residents through an independent lease agreement for their own bachelor apartment.
