Referral Forms and Information

For treatment as a hospital outpatient at the Western Campus call our office to see if you are eligible.  A physician referral is required.  Please see your specialist or family physician.  You may also see a doctor at a clinic.  Just be sure to have the referral with you at the time of your first appointment.  You may print out the attached referral form, but any signed, dated note which includes your name and diagnosis is acceptable.

The doctor's office may:

• fax the referral (519) 257-5299
• phone the referral (519) 257-5207
• send the referral by mail: Physiotherapy Services, 1453 Prince Road, Windsor, Ontario  N9C 3Z4
• ask you to call or bring the referral directly to the hospital.

If you have the referral yourself, you may:

• fax the referral (519) 257-5299
• phone the referral (519) 257-5207
• send the referral by mail: Physiotherapy Services, 1453 Prince Road, Windsor, Ontario  N9C 3Z4
• drop the referral off in person to the Physiotherapy Department

Once we have your referral, if you have been waiting for a while you are encouraged to call (519) 257-5207.

Patients who have had a recent surgery are given top priority.
