Privacy Commitment -Your Personal Health Information

Windsor Regional Hospital collects, uses, discloses, and retains personal health for the purposes of:

  • Direct patient care;
  • Administration and management of the health care  system;
  • Quality Improvement (such as satisfaction surveys) and risk management activities;
  • Research, teaching, and compiling statistics;
  • Fundraising (name and address only);
  • Complying with legal and regulatory requirements.

Your personal health information will only be used in compliance with Windsor Regional Hospital’s Privacy Policy (see attachment below).  If you wish to learn more about how your personal health information will be used and the security measures being taken to safeguard your privacy, please ask your health care provider.  Your health care provider may refer you to the Privacy Officer for more information (see attachment below).

Your request for health care services implies consent for the handling of your personal health information for specific, related purposes.  Unless a new purpose is required by law, your personal health information will not be used for a new purpose without your consent.

You may withdraw consent in writing at any time, subject to legal or contractual restrictions and reasonable notice.

Windsor Regional Hospital has guidelines and procedures with respect to the retention and destruction of personal health information, the details of which are available on request.

Upon request, you have a right of access to, and correction of, your own personal health information.  Windsor Regional Hospital can also provide an account of the use that has been made, or is being made, of this information and an account of third parties to which it has been disclosed.

Please be advised that we have experienced an inadvertent discarding of some personal and personal health information for patients that had passed away who would have been transferred to the morgue at the Ouellette campus (formerly Hotel-Dieu Grace Hospital) between approximately October 2010 and June 20, 2014. Information which may have been discarded may include such items as personal address, phone number, donor selection or O.H.I.P. identification number. Please be assured that Windsor Regional Hospital has notified the Information and Privacy Commissioner of Ontario of this occurrence and has reviewed our practices to ensure the safe destruction of information in accordance with the legislation.
